Bologna…first video from this guy that’s FOS…I finished med school not that long ago and there’s a clear difference in regards to med student aptitude and work ethic…yes, it can be hard but it does translate to knowing more and preparing for residency. I fear for the general public, my family, and myself and this current generation of med students are so sub-par that I am looking at other countries to move to so I can receive higher quality healthcare. Here, it is now a race to the bottom, which will become much worse if it goes full socialistic

ОтветитьBragging about working ridiculous overtime is like bragging about drinking gallons of alcohol. Forcing others to do the one is similarly like forcing others to do the other.
And forcing DOCTORS to be EXHAUSTED while making LIVE-DECIDING medical decisions.... is a recipe for a wrongful death suit.

ОтветитьI dunno it seems like such a fucked up world that we have to pay to become an extremely important, very urgently needed social foundation. Blows my mind man. I get it, not everyone is cut out to be a doctor. It's a fucking brutal job... but man the least we can do is give some breaks to them for learning. Even those who drop out or fail.
ОтветитьI think you nailed it with surgery. One of those types at A Big Teaching Hosp in NYC made me opt for vet med instead. I wish back then I'd been familiar with the famous Robert DeNiro line from Taxi Driver: "You talkin' to ME?!"

I often find you could interchange ‘doctor’ for ‘nurse’ or ‘midwife’ and same applies. Maybe not that much debt however. Keep up the awesome job

ОтветитьAbsolute facts. The sheer volume of information that needs to be digested, balanced against the ridiculously high expectations out of work as well is crazy. Most trainees (residents in the US) I've met do full time work, but also do research (college required or "getting onto the program" requirements), masters or phd and some sort of social life because somehow it's now an expectation as well.
"oh, doctors should have work life balance. Here's a new whole block of societal expectations around making sure you're a well rounded person while you're drowning in work. But if you don't balance work life, it's your own fault! Ladida~~"
The issue is also that everyone is high achieving in medicine. It's crazy - esp in some of the more competitive specialties. And everyone puts on a good front and smiles and social media life because THAT'S WHATS EXPECTED now - by bosses and coworkers and friends and families; meanwhile you're drowing in pressure and a too full plate while laughing with the consultants about "how it's crazy they had to do so much work before!! Omg our training is so much more doable, thank you for being supportive!" because it's the only way to survive in this place.
I'm always told that "it's not different in any other job. Suck it up."; but I find it difficult to believe it's common in other jobs to be 24/7 on call for a week, then work 8-5pm for another week, to get 2 days off and do it all over again; all the while balancing the job against more out of work work just to be competitive.

ОтветитьFrom conversations with my pharmacy resident fiancée, “Is there anything else we can do for you” is a not a question; it’s a challenge. The correct answer is, “no you’re free to go home.” Anything else that’s requires additional time is a borderline violation of the 4th Amendment.

I think a LOT of 'old' doctors need to see this, or something like it. There's a damn good reason working ridiculous hours should be banned, outright - it's extremely hazardous to doctor's health, and potentially lethal to patients.
The AMA 'Guidelines' state that:
"Maximum work hours: your hours mustn't exceed 75 hours in any 7 consecutive shifts, 140 hours in any 14 consecutive shifts, or 280 hours in any 280 consecutive days ..."
But between, hospitals squeezing, and the disaster that is COVID ... I don't think many doctors (or nurses) have seen 'reasonable' in several years!
The whole system will pay, and sooner, rather than later. 😔

ОтветитьAviation has the same sort culture, come out of flight school and get told you need to , "pay your dues." What they really mean is doing menial labour for a company in some freezing hole in the arctic for less than minimum wage for months or years before you are allowed to touch a plane. All while struggling with lines of credit and student loans, cost of living in remote locations and paying to relocate to some random ice-cave in a fly in only community.

ОтветитьUnfortunately, most studies have shown that the reductions in work hours have not translated in any improvements in outcome. It's swung a little bit too far from one extreme to the other. Now, instead of going over your patients the next day to dot all the i's and cross the t's, post call and going home a little bit early, residents are forced to abandon the service before rounds and the covering has absolutely no idea anything about the patient. Many residents and fellows in training actually don't like many of the more stringent rules because it forces them to take less care with their patients.

ОтветитьTrue not just for medicine, but for everything. The economy that boomers grew up in was massively government subsidized thanks to innumerable New Deal programs. High unionization rates also flourished, keeping workers powerful.
Housing & school were cheap, wealth inequality was low, & taxes were high.
Then they got old & decided they didn't want to pay the taxes that gave them a leg up & they broke the unions because they owned the businesses now. The so-called "Reagan Revolution" destroyed everything that created the American Golden Age all so the richest generation in US history could be just a little richer before they die.
I honestly love how the med students come to Bills aid. Bill is the gap between hard ass attendings who worked unnecessarily long hours and the new batch of interns and residents who want to change the narrative into a more balanced life. He has yet to figure it all out but the med students got his back.
Ответить....you all study medicine for only 4 years? In my country it's 6 years plus 2 years masters degree (optional) plus 2 years residency (mandatory)
Edit: I have been informed it's actually FIVE years of residency

ОтветитьOh my gosh, it’s too early for this. I worked in the hospitals in the later 70’s and it was a bit wild, although it was better for docs and patients and it wasn’t the business model it is now. The client bit…was true!

ОтветитьI think I am lucky in this case, though work hour is still extensive but my teachers/professors are really empathetic towards us. They always encourage us to pursue co-curricular and take time off or take stress down.
ОтветитьWork hour restrictions make sense. You don’t allow a pilot to fly planes if they are feteged, you don’t allow train and truck drivers to operate heavy vehicles. So why don’t you expect drs to make sound medical decisions?

ОтветитьI have watched my husband who loves teaching and was an attending work harder than the med students and residents because of the work hour restrictions. He just put in the long hours so that the students and residence can go home. They are protected by the new laws but the attending continues to take it like they did in residency without complaining. Work life balance does not apply to the attendings. He does not get paid as much to be in academia but devotes himself to training future doctors. I am very proud of him and when he retired, his students and fellows came from all over the country to attend his retirement party. One of his staff came up to thank me for supporting him in his long hours and dedication and I broke down crying. His students and fellows have been trained well to serve in their communities. I am a fan of Dr G but felt compelled to let you know that as an attending, my husband made sacrifices to give his students a work life balance.

ОтветитьBut coke is how you get all your charts done. If i achieve 100% what my ceo wants, i will raise the standard but quickly die. Then i'll be replaced by someone who will now need to live up to that deadly standard, and so on, meaning there will always be 100% achievement rate if i burn out, which is yay for human progress! 😀😀 If i die, others live, but if i live, others die.
